SUMMARY
The Clinical Outreach Coordinator (COC) is a centralized resource who partners with ProHealth providers and is part of the Clinical Operations Team. The COC provides outreach and education to patients transitioning from inpatient/outpatient settings. The role will focus on improving quality of care through patient engagement, practice engagement, gaps in care and barrier identification. Specifically, the Clinical Outreach Coordinator is responsible for ensuring “safe and effective care transitions,” addressing potential gaps in care, among other goals. He/she will communicate with providers, practice managers, and other team members to provide the necessary support to achieve optimum quality outcomes through outreach efforts, and care coordination. ProHealth Physicians of Wethersfield, a Pediatric practice, has an immediate opening for a friendly, customer focused and detailed oriented Medical Assistant to join our team.  The approximate hours will be from ­­­­­­8:15am – 5:15pm, M – F, with an occasional weekend day.

 

Position Description

The Certified Medical Assistant supports the Care team and its patients through patient care, clinical and administrative tasks including patient intake, receiving and responding to patient phone calls, and documenting patient interactions. The MA performs duties within their scope of practice delegated by, and under the supervision of, a provider or physician.

 

 

Primary Responsibilities:

  • Performs all duties within the scope of a Medical Assistant’s practice
  • Performs EKGs and other office testing
  • Performs quality control checks on equipment
  • Rooms patients according to policy and procedures, prepares patient for examination
  • Records patient care documentation in the medical record accurately and in a timely manner
  • Coordinates patient care as directed by physicians, company standards and policies
  • Respects patient confidentiality at all times and treats patients with courtesy and respect
  • Organizes exam and treatment rooms, stocks and cleans rooms and sterilizes instruments
  • Practices standard infection control precautions
  • Telephone and in-person screening limited to intake and gathering of information without requiring the exercise of judgment based on clinical knowledge
  • Carries out standing orders consistent with ProHealth’ s policies and procedures
  • Schedules tests, treatments and follow-up visits on behalf of patients
  • In advance of the visit, perform Pre-visit Preparation to confirm that all needed information is available
  • Informs new and existing patients/families about how to access information about their healthcare team
  • Delivers quality customer service
  • Ensures all policy and procedures are followed including infection control, patient confidentiality and privacy
  • Actively participate in regular team meetings and quality improvement activities
  • Responsible for conducting outreach to patients identified on monthly clinical performance reports
  • Provide effective communication with all patients
  • Performs all other related duties as assigned

 

Requirements:

  • High school graduate or GED equivalent required
  • Medical Assistant Certification required
  • Basic computer literacy required
  • Experience working with an electronic health record is preferred
  • Knowledge of medical terminology
  • Ability to react calmly and effectively in emergency situations
  • Good communication and customer service skills
  • Knowledge of ICD – 10 and CPT coding preferred

 

Physical & Mental Requirements:

  • Ability to lift, push or pull >35 lbs. with assistance
  • Ability to stand for extended periods of time
  • Ability to use fine motor skills to operate equipment and / or machinery
  • Ability to receive and comprehend instructions verbally and / or in writing
  • Ability to use logical reasoning for simple and complex problem solving
  • Occasionally requires exposure to communicable diseases or bodily fluids
  • Ability to discriminate shades of color when reading dipstick

ProHealth Physicians of Wethersfield has an immediate opening for a friendly, customer focused and detailed oriented LPN/RN to join our fun, pediatric team. The approximate hours will be 5 days a week, 8:30 to 5:30.

 

Position Description:

Support the care team and its patients by providing and documenting nursing care following established

standards and practices. Provide patient care and education and assist in locating and coordinating services from other providers and resources. Serve as liaison between patients, staff and providers. Document all patient interactions and other clinical activities accurately and in a timely manner in the

electronic health record (EHR).

 

Primary Responsibilities and Desired Behaviors:

  • Demonstrate respect for patients and ProHealth staff by presenting a friendly, courteous manner at all times and maintaining patient confidentiality.
  • Provide attention to patients in a manner that builds confidence, trust and loyalty.
  • Act as a member of the care team, including preparing patients to be seen by collecting and recording(as applicable):

-Chief complaint and history

  • Triage patients according to approved protocols and consistent with ProHealth’s Appointment and Scheduling Guidelines. Answer and document patient phone calls; take complete messages and route to providers as needed using the EHR.
  • Provide and document patient care such as giving injections and performing preventive screenings.
  • Educate patients regarding their illness or injury and provide instruction regarding self-management of their condition.
  • Carry out standing orders (i.e., authorizing prescription refills and ordering tests) consistent with ProHealth’s policies and procedures.
  • Schedule tests, treatments and follow up visits on behalf of patients. Assist patients with identifying suitable outside resources for disease management and other services; refer patients when appropriate.
  • As appropriate, conduct outreach to patients in need of appointments or other services, to communicate test results and other information, and to follow up after appointments to be sure patients are following their plan of care.
  • Monitor task list in order to respond to tasks in a timely manner, follow up on overdue orders and monitor status of critical referrals.
  • Order supplies and vaccines and maintain appropriate records. Prepare and maintain equipment including sterilization.
  • Conduct orientation of clinical support staff and provide training/coaching for staff related to clinical issues.
  • Maintain knowledge of current coding, OSHA and CLIA regulations and ProHealth policies.
  • Assist care team to assure smooth office operation and delivery of excellent service through teamwork.
  • Perform other duties as assigned.

Requirements:

  • Professional education for nursing and Connecticut RN licensure, BSN is preferred. Alternatively, completion of the licensed practical nursing educational program and Connecticut LPN licensure.
  • Requires knowledge of the practice specialty, e.g., pediatrics, internal medicine, family medicine.
  • Some nursing experience is preferred.
