The LPN Clinical Outreach Coordinator (COC) is a centralized resource who partners with ProHealth providers and is part of the Clinical Operations Team.  The COC provides outreach and education to patients transitioning from inpatient/outpatient settings. The role will focus on improving quality of care through patient engagement, practice engagement, gaps in care and barrier identification. Specifically, the Clinical Outreach Coordinator is responsible for ensuring “safe and effective care transitions,” addressing potential gaps in care, among other goals. He/she will communicate with providers, practice managers, and other team members to provide the necessary support to achieve optimum quality outcomes through outreach efforts, and care coordination.  The COC may also perform outreach to patients with chronic conditions or other specific care needs to inform them on the importance of having routine and diagnosis based testing performed to improve their overall health.

 

RESPONSIBILITIES:

Works with ProHealth providers and other team members to provide the necessary support to ensure safe and effective transitions and optimum quality care through outreach efforts:

  1. Actively manages ProHealth inpatient/outpatient hospital census lists to conduct transition in care outreach;
  2. Directly engages patients to ensure follow-up appointments are scheduled, discharge instructions understood, and medication adherence;
  3. Communicates as needed with providers, practice managers and other patient centered team members to update on barriers to care or gaps in care;
  4. Document transition care activities in patient electronic health record;
  5. Initiates second level review regarding complex patient needs with a RN Transition Care Coordinator;
  6. Engage patients using motivational interviewing techniques through phone calls or other methods;
  7. May be asked to participate in cross training in order to function in multiple roles as required;
  8. Collaborates with Case Managers and other staff, both in the central office and at practice/payer sites, to ensure outreach efforts are aligned and not duplicative.

 

QUALIFICATIONS:

  • Prior experience as a Licensed Practical Nurse in a healthcare environment, particularly in an ambulatory, physician practice setting, or health insurance environment is preferred;
  • Knowledge and experience with an electronic medical record is preferred;
  • Must possess strong interpersonal, organizational, and communication skills (written and verbal);
  • Must possess strong problem solving skills and attention to detail;
  • Ability to work well with teams in a variety of settings;
  • Must possess superior interpersonal skills and a highly evolved sense of customer service;
  • Strong computer skill set with working knowledge of Microsoft Office Suite (Excel, and Word) is required;
  • Ability to prioritize multiple tasks.

Our Field Based Care Managers are RNs who are responsible for the coordination of care for high risk individuals in the community setting to help keep them out of the hospital while remaining in a safe home environment with quality of life. The Field Based Care Manager acts as a longitudinal advocate who links the patient and care givers with the network of care team members to help them gain knowledge of their disease process and to identify community resources for continued growth toward the maximum level of independence. The Field Based Care Manager is responsible for care management activities across the continuum of care including coordination of care, development of a comprehensive care plan with any identified barriers, providing health education, coaching and treatment decision support for patients. The FBCM participates in interdisciplinary conferences to review clinical assessments, update care plans and determine follow-up frequency with the team.
Essential Functions
·         Collaborates effectively with interdisciplinary team to establish an individualized plan of care for patients’ goals including both short and long term·         Works with the Utilization Management RNs, Acute Care Engagement Specialists, and other Care Team members to facilitate smooth care transitions·         Stratifies and / or validates patient level of risk and communicates during transition process with Interdisciplinary Team·         Ensures standardized execution of workflow processes

·         Provides assessments of physical, psycho-social and transition needs in settings not limited to the Primary Care Physcian office, or patient’s home

·         Develops interventions and processes to assist the patient and/or caregiver in meeting short and long term plan of care goals

·         Field Based Care Managers work with their supervisor to work their assigned case load in an efficient and effective manner utilizing time management skills to facilitate the total work process

·         Confers with Medical Director(s) on a regular basis regarding cases and participates in departmental conferencing

·         Attends and participates in interdisciplinary team meetings as directed

·         With the assistance of the UM team, guides physicians and specialists in their awareness of preferred contracts and providers as well as facilities

·         Enters timely and accurate data into designated care management applications as needed to communicate patient needs and maintains audit scores of 85% or greater on a monthly basis as well as to be determined patient satisfaction

·         Consults with Medical Directors and/or other appropriate programs/resources to overcome barriers to meeting goals and objectives and presents cases to appropriate resources to obtain multidisciplinary view in order to achieve optimal outcomes

·         Maintain a comprehensive working knowledge of community resources, payer requirements and network services for target population to maximize benefit to patient overall well-being

·         Attend meetings as required

·         Other duties as assigned

Minimum Qualifications
 Education:    Associate’s degree requiredBSN preferred
