About Our Services
Primary Care for Your Family
Our providers have been serving the Manchester community for many years. For adult medicine, our office will 
see patients ages 14 and older. Dr. Minh Han and Dr. Monique Martin, both family medicine doctors, will see all ages, beginning with newborn care.
Our office is open Monday through Friday from 8AM-5PM. To make an appointment, please call (860) 533-4176.
Extended Hours
For your convenience, our office is open late on Wednesday with evening appointments available. Our office is also open on Saturdays from 9am-12pm.
Dr. Nair is available from 8:30am to 6pm on Wednesdays. Dr. Han is available from 1pm to 7pm on Wednesdays and from 9am-12pm on Saturdays.

Our records are fully electronic and available 24-hours a day in a secure, digitally accessible format. This allows us access to your complete and up-to-date medical records anytime, from anywhere, making it easy to coordinate patient care with other doctors and specialists. It also gives us the ability to prescribe medications electronically, saving you time at the pharmacy.
