Clinical Performance

ProHealth’s clinical performance team develops and implements a broad array of clinical initiatives that help our physicians provide the best possible care to our patients. Our programs are designed to identify opportunities for improving clinical outcomes in key quality areas such as diabetes, coronary artery disease, immunization against disease, and overall prevention.

ProHealth At A Glance: Clinical Performance

ProHealth applies a comprehensive approach to improving clinical performance. We understand that the relationship in the exam room is between the provider and the patient. However, we also recognize the level of support that a provider needs to optimize the value of that time. Our clinical performance staff equips our providers with the tools, processes, and policies that are required to identify whether a patient meets the criteria for particular clinical conditions as well as whether the patient’s results indicate the need for more immediate intervention or routine follow-up.

ProHealth’s Clinical Performance Services include:

  • Monthly provider reports that illustrate a provider’s individual results on industry-accepted quality measures
  • An electronic database tool that enables office staff to enter and track key clinical results, such as laboratory tests and screenings
  • A physician-led Clinical Performance Committee that develops policy and procedure protocols and oversees activities related to clinical initiatives
  • Formal interaction with ProHealth’s Practice Management division to develop and implement best practice processes and workflows that are designed to facilitate improved outcomes and increased efficiency at the practice site
  • Disease management reporting that outlines key clinical metrics relative to each patient identified as having met the criteria for the disease state
  • Generic prescribing reports

ProHealth’s Clinical Performance Results:

ProHealth received an award from the National Committee for Quality Assurance (NCQA) Diabetes Physician Recognition program, which recognizes our commitment to exemplary care that is consistent with evidence-based guidelines for managing diabetes. Our wellness and disease management programs have:

  • Reduced the percentage of our patients who have poorly controlled diabetes
  • Increased our immunization and wellness visit rates for the pediatric and adolescent populations
  • Improved our cholesterol management results in both the coronary artery disease and diabetic patient populations
  • Increased the colorectal cancer screening rates for men over age 50

While we are pleased with our results, we always strive for being the best. Looking forward, we will:

  • Collaborate with our developing cardiology network to implement evidence-based protocols for diagnosing and managing common cardiovascular conditions to improve patient outcomes
  • Enhance access and availability by offering extended office hours in each community
  • Ensure our patients have access to clinically appropriate generic alternatives to help manage the patient’s cost for required prescription medications
