March Primary Care Coalition Meeting

Our next Primary Care Coalition of Connecticut (PCCC) meeting will occur on March 27th at the ProHealth Administrative office in Farmington from 6:30 p.m. until 8:30 p.m.
We are excited to learn about our topic, which is Building a Community to Support Frail Elders, and to discuss ways that we might all collaborate to better serve this growing segment of the population. As the wave of baby boomers reach senior citizen status, it is critical that the variety of organizations who serve frail elders work closely together to meet their needs in a cost-effective and coordinated way.
Our speaker is Dr. Robert Schreiber, a geriatrician from Harvard Medical School and Medical Director of Evidence-Based Programs at Hebrew Senior Life. Rob has extensive experience working with healthcare and community service providers, government agencies, organizations that serve the underserved, including those with disabilities, patient advocacy groups, and many others to achieve this aim. We look forward to participating in a lively discussion on this topic with you next week.
