We're moving to MyChart in February 2024

We’ve got exciting news to share. In February 2024, ProHealth Physicians is moving to a new electronic health record (EHR) system called Epic. This includes your new patient portal, MyChart. All ProHealth patients can join MyChart on February 5, 2024. 

There are many benefits to signing up for MyChart, such as helping you manage your health with ease and convenience. For starters, you’ll be able to:  

  • Connect with your ProHealth clinician, no matter where you are
  • Make or cancel appointments without calling your doctor's office
  • See upcoming appointments, medications, test results and medical bills
  • Ask for prescription refills

With MyChart, your ProHealth care team will have your health information all in one place — even if you've been seen at multiple health care organizations.* It's a one-stop shop for your health care needs.

*This applies to health care organizations using Epic electronic health records in Connecticut.

Important dates

If you use FollowMyHealth, you'll need to sign up for MyChart to stay connected with us.

Key dates and information:

  • On January 19, you won't be able to sign up for FollowMyHealth anymore. We'll be getting ready for the switch to MyChart.
  • On January 26, we'll start moving to MyChart and the FollowMyHealth portal will stop updating. You won't be able to send messages to your ProHealth clinician using FollowMyHealth. We will only answer messages sent before January 26. If you have questions about your care, please call your doctor's office directly. If it's an emergency, call 911.
  • On January 29, patients will begin getting emails with codes to sign up for MyChart. People visiting our offices will be asked to sign up when they come in. Note: It will take us a few weeks to fully switch to the new system. During that time, some features and health records may not be available yet.
  • On February 5, ProHealth will be live on MyChart. We'll continue sending emails with activation codes to sign up. If you don't get an email, you can sign up in person at your doctor's office. We'll ask for identifying information, including the last 4 digits of your Social Security Number. This is how we make sure it's really you. Once confirmed, we'll send you an email with a sign-in link for MyChart.

Please be patient while our ProHealth care teams learn the new system. We're here to help you through this change.

Frequently asked questions

  • No, you will not automatically have a MyChart account. You will need to sign up for the new portal. It's quick and easy to sign up for MyChart. 

    Patients will be able to sign up doing one of the following: 

    • Email: You may get an enrollment email with a sign-up link. This link is your access to register for a MyChart account. The portal invitation will come from donotreply@prohealthmychart.com. The subject line will say, "You're invited to create a Patient Portal account with ProHealth Physicians." If you don't get an email, check your junk folder.
    • Online: Didn't get an email? You will be able to complete our online form to request MyChart access (ages 18+). It can take up to 48 hours to verify your account. You will get an email when it's available.
    • In person: You can also sign up for a MyChart account with our reception staff in the office. No appointment needed.

  • If you have a current myProHealth account, you will be able to login to view health information only. You will no longer get health history updates or be able to message your clinician using this portal.

  • Once we are live, there will be plenty of ways to sign up.

    How to sign up for MyChart:

  • Starting on February 5, you can use our online form to request your MyChart account (age 18+). To make a MyChart account for someone else (called a proxy account), please call their ProHealth office and press 4 for help.

  • You will not lose any health information. You may not see everything right away. Over the next few months, your complete ProHealth medical health history will move to MyChart.

  • MyChart is a host application. Each health organization has their own portal within MyChart. Even if you signed up for MyChart with another group, you still need to join our ProHealth Physicians portal to see your health information with us.

  • Parents, caregivers and legal guardians. You will have the ability to see the health information of the patient you represent through your own personal MyChart account, this is called proxy access. With proxy access, you can do almost all the same actions as you would with your own account. Though there are some things to be aware of depending on the child's age.

    • Ages 12 and under: Parents, caregivers and legal guardians can:
      • Send messages to their child's doctors and health care providers
      • Schedule appointments, including well-child checks
      • View upcoming and past appointments
      • Pay bills
      • View immunization records, test results and more
    • Ages 13–17: Once a child turns 13, there are some changes to what parents, caregivers and legal guardians can see in a child's MyChart account. You can still pay bills and view immunization records. However, sensitive information related to appointments, test results and medications won't be available to protect the child's privacy. This offers teens a safe space to seek medical care and guidance.
    • Ages 18 and older: Once a child turns 18, parents, caregivers and legal guardians no longer have access to their child's health record in MyChart. You will have to be granted access by the adult child. A person 18 years old or older can grant access from their own MyChart account by going to "Share MyRecord Activity," selecting "Friends and Family Access," and clicking "Invite Someone."

  • Yes, patients with a ProHealth Physicians MyChart account can book an appointment with us using the portal. If you do not have a MyChart account, you can sign up or book as a guest.

  • Yes, patients will be able to pay their medical bill directly through MyChart. To pay a current bill/bill for a service before February 5, 2024, visit our Pay a Bill page.

  • Almost all test results will be released right away once they're available. This means you may see your results before your doctor. When your doctor does review your results, they may add a comment in your account. This will then create an update to the result in MyChart. Some results may be delayed at the discretion of your doctor. As always, if you have questions or concerns, please call your office.

  • If you have a current FollowMyHealth account, you'll still be able to log in and view your ProHealth medical history. You will no longer get health history updates or be able to message your clinician using this portal.

  • This time allows you to complete your paperwork without feeling rushed. So, please do the best you can to come a bit early.

  • Epic is the entire electronic health records system. MyChart is only the patient portal supported by Epic.
