Billing policies
Payment is expected at the time of service. If you have health insurance from a provider we work with, we will submit a bill. Please make sure your insurance information is correct and complete. This helps speed up the payment process.
After your insurer processes your claim, we will bill you for any remaining balance. Your billing statement will explain what you need to pay and why.
If you have questions, please call customer service toll free at 1-800-924-7952. You can also email us at ProHealthPhysiciansCT@optum360.com. A representative will review your account and get back to you quickly.
We send billing statements by mail every month. Prompt payment is appreciated.
You can make your payment in several ways:
Pay online
MyChart is the easiest way to pay your bill, make appointments, see results and more. Pay online using MyChart. Not ready to sign up? Pay as a guest with MyChart using your account number and last name.
Pay by phone
Call 24 hours a day, 7 days a week to pay your bill by phone with a credit card: 1-800-924-7952, option 1.
Have billing questions or need a payment plan? Call Monday–Friday, 8 a.m. to 4:30 p.m. ET and press option 2.
Pay by mail with check
Fill out the form at the bottom of your paper statement and return it with your check in the envelope provided. Please write your account number on your check and make it payable to ProHealth Physicians.

Please be prepared to pay an unpaid balance or copay when visiting our offices.
If you are uninsured or a self-pay patient, you have the right to receive a good faith estimate of expected charges before receiving services.
Please let us know as soon as possible if you can’t keep your scheduled appointment. We ask that you call the office at least 4 hours before your appointment time. This lets us give the appointment to someone who might urgently need it.
If you need to fill out medical forms for school, camp, work or other activities, the staff at your doctor’s office can help. A fee may be charged for this service. For information, contact your doctor’s office.
