I gave this rating as she had false information on my visit details. I literally told her I do not drink or smoke and she still put “Alcohol consumption is reported”on my visit details. I never had a MD put false information on my visit details before, and I had to call the office to have my details corrected. This is highly unprofessional and that was my first and last time have a physical exam with Alicia!
