Making sure you and your family get the COVID-19 vaccine is one of our priorities. We know you have many questions about the vaccine, eligibility, where and how to book an appointment, and more. This step-by-step guide will help answer them.

Step 1: Check your eligibility.

All individuals who are 5 years of age or older and live, work, or attend school in Connecticut are eligible to receive the COVID-19 vaccine. Learn more from the Connecticut Department of Public Health (CT DPH).

Step 2: Book your COVID-19 vaccine appointment.

We have the Pfizer® COVID-19 vaccine in stock at nearly all of our pediatric and adult primary care offices. ProHealth patients who are unvaccinated or meet the requirements for a booster shot may be able to get the Pfizer COVID-19 vaccine at an upcoming visit.

You can also find a statewide list and map of COVID-19 vaccine clinics through 2-1-1 of Connecticut. You’ll enter your ZIP code or town in the location box on the right. Then, press the yellow search icon.
