Our billing department staff is as committed to high-quality service as our care teams are. We make sure that medical claims are paid quickly and correctly by your insurance provider. We’re also here to help if you have questions.

If you are an uninsured or a self-pay patient, you have the right to request a good faith estimate of expected charges before receiving services.

  • Payment is expected at the time of service. If you have health insurance from a provider we work with, we will submit a bill. Please make sure your insurance information is correct and complete. This helps speed up the payment process. 

    After your insurer processes your claim, we will bill you for any remaining balance. Your billing statement will explain what you need to pay and why.

    If you have questions, please call customer service toll free at 1-800-924-7952. You can also email us at ProHealthPhysiciansCT@optum360.com. A representative will review your account and get back to you quickly.

  • We send billing statements by mail every month. Prompt payment is appreciated.

    You can make your payment in several ways:

    • Online
    • ­By credit card over the phone; call billing customer service toll free at 1-800-924-7952
    • In person at any ProHealth Physicians office*
    • ­Mail the top section of your statement with credit card information or a check made payable to ProHealth Physicians to our mailing address:

    *As of March 1, 2022, we will no longer accept payment by cash or check. Please be prepared to pay an unpaid balance or copay by credit or debit card when visiting our offices. We accept credit and debit cards from Visa, Mastercard and Discover, including HSA, FSA and prepaid cards.

  • Please let us know as soon as possible if you can’t keep your scheduled appointment. We ask that you call the office at least four (4) hours before your appointment time. This lets us give the appointment to someone who might urgently need it.

  • If you need to fill out medical forms for school, camp, work or other activities, the staff at your doctor’s office can help. A fee may be charged for this service. For information, contact your doctor’s office.
