  • Payment is expected at the time of service. If you have health insurance from a provider we work with, we will submit a bill. Please make sure your insurance information is correct and complete. This helps speed up the payment process. 

    After your insurer processes your claim, we will bill you for any remaining balance. Your billing statement will explain what you need to pay and why.

    If you have questions, please call customer service toll free at 1-800-924-7952. You can also email us at A representative will review your account and get back to you quickly. 

  • We send billing statements by mail every month. Prompt payment is appreciated.

    You can make your payment in several ways:

    • Online.
    • Mail the top section of your statement with credit card information or a check made payable to ProHealth Physicians. The mailing address is P.O. Box 419745, Boston, MA 02241-9745.
    • By credit card over the phone. Call customer service toll free at 1-800-924-7952.
    • In person at any ProHealth Physicians office. Payment may be made by cash, check or credit card.
  • Please let us know as soon as possible if you can’t keep your scheduled appointment. We ask that you call the office at least four (4) hours before your appointment time. This lets us give the appointment to someone who might urgently need it.

  • If you need to fill out medical forms for school, camp, work or other activities, the staff at your doctor’s office can help. A fee may be charged for this service. For information, contact your doctor’s office.
