As part of our care management program, we regularly call patients with serious or chronic health conditions. It’s a way for us to make sure you’re staying on top of your care. It’s also a chance for you to ask questions and get the support you need.
Simple though this may sound, outreach calls help patients improve their health. All calls are made by nurses or trained service representatives.
Reasons for calls include:
- Making sure you’re taking care of yourself if you have a serious or chronic condition, like heart disease or diabetes
- Reminding you that it’s time for a preventive exam, health screening or test
- Scheduling follow-up care if you recently had surgery or were hospitalized
- Helping you find ways to be actively involved in your care

Once you’re in our care management program, you'll automatically receive outreach calls. It’s part of our commitment to helping you get the care you need, when and where you need it.
